This show starts out with a scene from the past – four years ago, to be exact. Sam and Dean had captured a demon that they needed to draw names from in order to track down Lilith. With the help of a woman from the area, the two brothers are able to get the names of Lilith’s next target. It involved a lot of torture and afterwards, Dean banished the demon to hell and took the man that was left to the emergency room.
The show moves forward to the present and we find Sam and Dean in the same town again. Dean is trying to get more information out of Frank about Dick Roman and the Leviathans. Sam is dealing with his own visions of Lucifer. He’s decided to hide those visions from Dean, though, and eventually he figures out how to make the devil leave.
There’s another issue on the brothers’ plate, though. The demon that they sent to hell four years ago has returned. There are the same types of murders happening again. The brothers find the same detective working the case that was there four years ago. Sam is being visited off and on by Lucifer, but he’s about to find sulfur in one of the victim’s cars.
Sam and Dean then head out to find the woman that helped them the last time they were in town. She decides that she doesn’t want any part in it this time and leaves town. However, she does remind the boys to check out the man the demon possessed the last time he visited.
Jeffrey has tried to return his life to some sense of normalcy and he’s actually doing fairly well. He’s surprised by Sam and Dean in an alley. When Dean tells Jeffrey that the demon has returned again, he’s scared, too. Jeffrey remembers a list of names that the demon was repeating four years earlier. Sam is dealing with Lucifer again and it’s getting harder to make him disappear.
Dean heads off to the woman who is next on the demon’s list. Sam accompanies Jeffrey back to his apartment and learns what really happened four years earlier with the demon. Jeffrey’s encounter with the demon left him in a mental institution, but he has slowly managed to recover.
This time, though, Jeffrey wants to help fight the demon. He remembers a place that the demon might go. Focusing on the task at hand is getting harder for Sam because Lucifer won’t give him any peace.
When Sam and Dean arrive at the location given to them by Jeffrey, they find all kinds of demonic symbols inside. There’s also a body that’s tied to a chair. Dean moves to release the man and ends up getting stabbed in the neck with a needle.
Sam eventually realizes that the symbols are part of a spell that was cast by the ever friendly Wiccan, Laura. Sam breaks into her store hoping to find some answers, but she doesn’t give up much. Lucifer tells Sam to go with violence, but Sam decides to only yell at Laura. She finally gives up some information and Sam learns that Laura is performing one of the most powerful summoning spells there is. The main ingredient in that spell, though, is Dean.
Jeffrey works quickly to collect Dean’s blood. The demon is back, but Jeffrey is not the host this time – Laura’s son is. The demon doesn’t want Jeffrey’s body this time, but he certainly wants Jeffrey’s service. Sam has convinced Laura to help him get rid of the demon and they arrive at the warehouse. Jeffrey tries in earnest to save the demon, but Dean ends up shooting him. Laura manages to send the demon back where it came from.
Back in the motel room, the brothers are exhausted. While Dean crashes out on the bed, Sam finds he’s unable to sleep because Lucifer is present and taunting him again.
